Preparing for adulthood in Ealing
A transition checklist for education, benefits, health, adult social care, decision-making, housing and travel before age 18.
What is it?
Support does not all transfer automatically at 18. Education, health, benefits and social care can use different ages, rules and teams. Starting early creates time for assessments, applications and a clear handover plan.
Who may qualify?
Relevant to young people with SEND or a disability who may need continued education, health, care, employment, independent-living or transport support. If there is an EHCP, preparing for adulthood should form part of reviews from Year 9 onwards.
What to do next
Create one shared transition list with names and dates. Ask the school or SENCO about the EHCP plan; ask health teams who will take over; request an adult social care assessment early if needs may continue; check PIP at 16 and appointeeship; review travel, college, work and decision-making arrangements. Record who owns every action.
What if there is a refusal or problem?
If a service says the young person is too young, too old or belongs to another team, ask for the decision and correct referral route in writing. Raise missing transition actions at the EHCP review and use the complaint or appeal route that matches the service.
